(a) A separate entry form must be filled in for each entry.
(b) Entry fees will not be refunded.
(c) By entering this Festival, you are giving permission to have your name and photo published in any media reports of said Festival. If you do not give permission to have photo taken please indicate here. ________
(d) The official Program is prepared from information submitted by you

E-transfer payments for entry fees can be sent to stirlingfestival@gmail.com or cheques can be mailed to 17-25 Heartwood Dr., Belleville Ont. K8P 5M2 with cheque made payable to Stirling Festival of Sacred Praise.
